Driver Release Form
The Driver Release Form can be downloaded here.
When provided, participants will be expected to ride district-sponsored transportation to and from away activities. With approval by the coaching staff, a student may ride with a parent to and/or from an away activity. For a student to drive themself or ride with a person other than a parent/guardian, a release form must be signed by the parent/guardian with the date and description of the activity. Complete the following form and bring it to the coaching staff the day of the activity.
